About The Role
This is a part-time, specialist role (1 day per week - Wednesdays) working as an independent practitioner to support a defined cohort of ophthalmology patients with diabetes.
The Diabetes Clinical Nurse Consultant will focus on improving patient engagement, self-management, and attendance at scheduled appointments and procedures within Specialist Clinics. The role is central to optimising patient outcomes by supporting individuals to better manage their diabetes and associated eye disease, reducing avoidable delays in care.
Working closely with ophthalmology, nursing, and allied health teams, you will provide targeted education, coordination, and follow-up for patients at risk of non-attendance or poor disease control.
